Palladium Care, LLC provides the following services:

Programs:

Activities and programming, which are provided by Palladium Care, will be at the level and frequency needed by each resident to provide needed stimulation. Activities will be chosen in accordance with resident capabilities and interests whenever possible. Activities and/or programming may be done individually with resident and staff or as a group with staff and home residents.

Resident Admissions & Policy

At Palladium Care, LLC,  we understand that finding quality Assisted Living can be a painfully time-consuming task. Often it is a time of anxiety and concern over finding the best place for your family member to stay. You are looking for someone who can understand the needs you have and someone who is able to assist in meeting those needs.
Palladium Care Adult Family Home will partner with you in order to accomplish assisted living goals. We take time during your initial visit to listen to your concerns. We understand the importance of the type of home environment and we seek to provide a family atmosphere for each of our residents.

 

We understand that assisted living is sometimes perceived as the "last resort" when considering options for living arrangements. We hope to change this perception by providing quality care in modern, spacious and well-kept home.

Whether you are entering assisted living for the first time, or transitioning from other living arrangements we will work with you to ensure your experience with us is pleasant and productive.

Admissions:

Individuals will be admitted to Palladium Care, LLC  regardless of religion, race or national origin. No one will be excluded for participating in activities, denied services or discriminated against in anyway. Restrictions may apply due to court orders and/or refusal of care and services by the resident and/or their responsible party. 

Prior to admission acceptance, the prospected resident will be evaluated by qualified staff to ensure compatibility with other residents. Maintaining and ensuring all persons safety and well-being is continually respected. As with new medications, progressing conditions or overall daily moods, situations may arise where interventions need to be used in controlling a behavior or episode. Behaviors must be manageable and should not require physical or chemical restraints for safety. 

Admission Policy:

  •  A pre-admission assessment is required, completed by one of the directors.

  • A physical exam, communicable disease screening, and TB test are required within 90 days prior to admission, or within seven days after admission in only upon urgent placement. Performed by a Physician, Physician's Assistant, Clinical Nurse Practitioner, or a Registered Nurse. The exam, screening, and test are required annually; the exam may be waived if the resident is seen routinely by a Physician at least every six months. The TB test must be performed once every year. Cost of the exam  and screening is the resident's responsibility.

  • Individual Service Plan (ISP)must be completed and implemented within 30 days of admission, updated and/or reviewed at least every six months. A care plan meeting will be held with involved parties at least every six months in order to assure the ISP was/is completed in accordance with the best interest, health and welfare of the resident.
